Please be advised on September 3, 2024, the GRC (Government Records
Council) of New Jersey updated the OPRA request form to reflect changes
made to the records laws. I have attached the form for your review and ask
that all future requests include the certification of questions 1, 2 & 3.

Please reply back to this email with answers to the following questions.

1. Have you ever been convicted of an indictable offense under the laws of
New Jersey, any other state, or the United States?

 2. Will the requested government records be used for a commercial purpose
