Records requested:
I am requesting open/closed permits and/or violations from January 2000 to present, as well as the Property Card. Also, I would like to know information regarding the presence of an underground oil tank and/or septic tank, if applicable, including permits, remediation reports, environmental reports, and surveys showing underground oil tank and/or septic tank location.

In response to the OPRA requested received by me on August 20, 2019 please find the following:
Open/Closed permits: Attached. There are two open permits 2004 a re-roof and 2004 Hot Water heater. It should be noted that the Hot Water Heater was replaced again after the 2004 permit and that was closed out.
Property Card: Attached.
Underground Oil Tank: We found no records responsive to this request.
