Dear Berkeley Heights Township,
Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-6(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.
I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.
I WILL NOT use the requested government records for a commercial purpose.
I AM NOT seeking records in connection with a legal proceeding.

2. Copies of resumes submitted for all candidates who applied for Sergeant from 2017 to 2024.
3. Copies of cover letters submitted for all candidates who applied for Sergeant from 2017 to 2024.
3. Copies of all Trait Assessments completed by supervisors for the Sergeant Exams from 2017 to 2024.
4. Copies of all Personnel Assessment sheets completed by candidates for the Sergeants Exam from 2017 to 2024.
5. Final scoring sheet for Sergeants Exam from 2017 to 2024.
6. Copies of all completed training certificates for all sworn police officers from 2017 to present.
7. List of all training completed by sworn police officers from 2017 to present to include all completed PowerDMS training.
8. Letters of Intent and resumes for Lieutenant, Captain, and Chief of Police for all members who submitted for those positions from 2017 to present. This pertains to the items that were submitted to the Union County Prosecutor’s Office during the Internal Investigation of Chief Jason Massimino. See page 2 of Union County Prosecutor’s Office Internal Investigation Case # EA-23-0016 item 6 for reference of requested documents.
