NJEDA's "Sustain and Serve" program- Ridgewood HealthBarn Foundation
Dear New Jersey Economic Development Authority,
This is a request for public records made under OPRA and the common law right of access. I am not required to fill out an official form. Please acknowledge receipt of this message.
I would like to request the following records:
New Jersey Economic Development Authority's "Sustain and Serve" program.
Regarding Applicant/Recipient- Ridgewood HealthBarn Foundation/ HealthBarn Foundation/ HealthBarn/ HealthBarn USA
Pertaining to this grant, please provide the following:
1. All applications and supporting documentation submitted
2. Applicant’s required list of all restaurants they wish to purchase meals from with Sustain and Serve NJ funding.
3. Form completed by listed restaurants used to verify eligibility
4. SSNJ Restaurant Interest Forms
5. Grantees dated receipts and invoices submitted for reimbursement
6. Expenses incurred from date of grant execution through present.
Time period 2020 to present.
