Sorry Timothy, but the policy is contrary to the law. A district cannot require use
of a particular form.

I am sorry to be a stickler on this but I have always requested this form
of all OPRA requests - It is something that helps us keep track of all
requests. And, it shows cost, if any, and approval on my part so we have
a written record.
I will again ask that you complete this form. It is very simple to
complete. If you don't agree, you of course have avenues including
complaining to the GRC. Again, not trying to be difficult, just trying to
treat all requesters equally.
