Insurance Carriers/Decarations Pages
Dear Medford Township,
This is a request for public records made under OPRA and the common law right of access. I am not required to fill out an official form. Please acknowledge receipt of this message.
Records requested: All insurance policy declaration sheets displaying the carriers/TPA's names, addresses and coverages to include policy limits and lines of coverage related to General Liability and Public Officials coverages for the policy periods for 2021/2022.
This includes paper and/or electronic documentation. As the funds of the public are utilized to purchase these coverages for the township; this request falls within the scope of an Open Public Records Request in the State of NJ.
Please allow this email to serve as a formal request under OPRA and the right to public access in the interest of transparency.
