This is a request for public records made under OPRA and the common law right of access. I am not required to fill out an official form. Please acknowledge receipt of this message.

Closed locations: Please provide copies of the health license and all inspection reports and complaints from 2000-present for the following closed locations:
1. Brandy's Tavern, 1380 Ocean Avenue, Lakewood NJ
2. Caneda's White Rooster, 781 Fischer Boulevard, Toms River NJ
