RECORDS REQUESTED:
For the period January 1, 2015 to present, please provide the following records concerning the provision of Emergency Medical Services (EMS) to Mansfield Township, including but not limited to services provided by Independence First Aid Squad:

1. All contracts, agreements, memoranda of understanding (MOUs), or other written arrangements between Mansfield Township and Independence First Aid Squad, including any amendments, renewals, or extensions.

2. All requests for proposals (RFPs), bids, solicitations, or competitive contracting documents relating to EMS services, including all responses submitted.

3. All resolutions, ordinances, meeting minutes, and agenda materials of the Township Committee referencing, approving, discussing, or ratifying:

(a) the transition from any prior EMS provider or structure to Independence First Aid Squad;
(b) the outsourcing, regionalization, or consolidation of EMS services;
(c) funding, appropriations, or payments for EMS services.

4. All correspondence (including emails and attachments) between any Township official, employee, or agent and Independence First Aid Squad relating to:

(a) EMS service provision;
(b) response times;
(c) staffing levels;
(d) coverage areas or mutual aid;
(e) performance concerns or complaints.

5. All documents reflecting performance metrics or service standards, including but not limited to:

(a) response time requirements or targets;
(b) actual response time data or reports;
(c) compliance reports, audits, or evaluations of EMS services.

6. All documents reflecting payments, subsidies, stipends, or other financial support provided by the Township to Independence First Aid Squad, including budgets, vouchers, and payment records.

7. All insurance, indemnification, or liability agreements relating to EMS services.

8. All internal Township analyses, reports, studies, or memoranda evaluating:

(a) the costs and benefits of maintaining a local EMS unit versus outsourcing or regionalizing services;
(b) the impact of EMS structure on response times, coverage, or public safety.

9. All complaints, incident reports, or internal reviews concerning EMS response times or service performance within Mansfield Township.

10. All documents identifying the EMS provider(s) serving Mansfield Township prior to the current arrangement, including any records reflecting the discontinuation, dissolution, or replacement of any prior local EMS entity.

If any portion of this request is denied, please provide a written statement of the specific legal basis for each denial, including citation to the specific exemption(s) relied upon.
If records are withheld or redacted, please provide a privilege log identifying the record, the date, the author, the recipient, and the basis for the redaction or withholding.
