2) salary/rate of compensation
3) payroll records sufficient to show total compensation paid out each year since the hire
4) start date for the employee
5) promotion date(s) for final title/position held and each title/position held previously
6) date of separation from employment and the reason for such separation
7) the amount and type of any pension received
8) if an employment contract exists for this employee, a copy of the contract
