Dear Lakehurst Borough,

Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-6(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

I WILL NOT use the requested government records for a commercial purpose.

I AM NOT seeking records in connection with a legal proceeding.

Records requested:
1. Any open or closed permits pertaining to the property.
2. Any documents relating to any variance pending, granted, or denied.
3. Complaints filed by the town and any other citations for code or other violations.
4. Any pending or finalized assessments for the property.
5. The tax assessor’s card, reports, or memos regarding the property.
6. Any easements or other restrictions of record.
7. Inspections for environmental issues (water wells, sanitary systems, or underground storage tanks).
8. Current Certificate of Occupancy (if applicable).
9. Any documents indicating if the property is in a flood zone.
10. A list of any offsite conditions that may affect the value of value property that is maintained by the town pursuant to the “Offsite Conditions Disclosure Act”.
11. Any “As-Built” drawing for septic system (if applicable).
12. Any and all information regarding a well on the property (if applicable).
