Dear Elizabeth City,
Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-5(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.
I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.
I WILL NOT use the requested government records for a commercial purpose.
I AM NOT seeking records in connection with a legal proceeding.

Good afternoon,
This is the first time I am hearing about this OPRA. If you want to send
us paper OPRAS, please send them to [email address]. I
just also wanted to inform you that as of January of 2025, we started
using GovPilot for all our OPRAS, so if you want to submit through
GovPilot, you will automatically get a confirmation number with it.

Dear City of Elizabeth,
Thank you for the record but it does not address my request. The IAPP requires police agencies to publish two documents on an annual basis: Appendix K (Internal Affairs Summary Report) and Appendix L (Major Discipline Report). Each of the documents is supposed to be published to the internet website of the agency no later than January 31 for the prior calendar year. Please provide those records: Appendix K and Appendix L.
