Dear Mr. McIntyre,
On behalf of Custodian of Records Sandy Patterson, please know that your
request for "the person or persons responsible for collecting, recording, and
imputing the results of the spring/summer athletic survey from the Jackson
Township High school" is not subject to OPRA, as it is a request for information
(rather than a document or record).
However, in the spirit of OPRA and transparency, we can answer your question
here and let you know that the survey, which was a Google Form, was administered
and the results collected by our Athletic Director Gregory McClain, in
coordination with district administrators.

*Please note the district is closed on Fridays in the summer.
