Requesting record request on ticket number 825-2608
Dear Logan Township,
Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-6(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.
I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.
I WILL NOT use the requested government records for a commercial purpose.
I AM NOT seeking records in connection with a legal proceeding.
Records requested:
1. A complete copy of the traffic ticket/summons issued.
2. All police reports and investigation reports associated with this ticket.
3. All officers’ notes, reports, supplements, and incident logs related to this ticket.
4. All video footage, body-worn camera recordings, dash camera recordings, surveillance video, and any other audiovisual recordings connected to this matter.
5. Any and all photographs or evidence files associated with this incident.
ADA Accommodation Request:
In addition, I request that you provide the contact information (name, phone number, email) of the ADA Coordinator or Title II ADA contact person for the Logan Township Municipal Court or the New Jersey Courts responsible for coordinating reasonable accommodations under the Americans with Disabilities Act (ADA). I require appropriate ADA accommodations in connection with this matter and future court
