Dear Perth Amboy City,
Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-6(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.
I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.
I WILL NOT use the requested government records for a commercial purpose.
I AM NOT seeking records in connection with a legal proceeding.
Records requested:
I am requesting copies of the following specific government records related to the Perth Amboy Animal Shelter. All requests below cover the time period January 1, 2024 through November 20, 2025.
1. Records Showing TNR Program Budget and Expenditures
Copies of any and all records reflecting funds budgeted, allocated, or designated for the Trap-Neuter-Return (TNR) program, as well as any and all records reflecting expenditures, disbursements, or spending related to the TNR program.
This includes, but is not limited to:
-Line-item budgets
-Financial ledgers
-Expenditure or disbursement reports
-Purchase orders
-Invoices
-Monthly or annual financial summaries relating to TNR activities
2. Records of Spay/Neuter Appointments Issued
Copies of any and all records documenting spay/neuter appointments scheduled, issued, or provided by the Perth Amboy Animal Shelter.
This includes, but is not limited to:
-Appointment logs or scheduling sheets
-Spay/neuter procedure records
-Monthly or annual spay/neuter reports
-Any records indicating which outside TNR groups received spay/neuter appointments and the number provided to each (if such records exist)
3. Records of Dead-Animal Pickups
Copies of any and all records documenting deceased-animal pickups conducted by the Perth Amboy Animal Shelter or associated animal control services.
This includes, but is not limited to:
-Dead-animal pickup logs
-Call sheets
-Dispatch or response records
-Monthly or annual summary reports tracking deceased-animal pickups
4. Records of Animal Euthanasia
Copies of any and all records documenting the euthanasia of animals by the Perth Amboy Animal Shelter.
This includes, but is not limited to:
-Euthanasia logs
-Veterinary records relating to euthanasia
-Monthly or annual euthanasia summaries
5. Records of Cat Intake and Cat Adoptions
Copies of any and all records documenting feline intake and feline adoptions at the Perth Amboy Animal Shelter.
This includes, but is not limited to:
-Intake logs
-Adoption logs
-Monthly or annual shelter activity reports reflecting cat intake and adoption statistics
If any portion of this request is unclear, or if any records are exempt or partially exempt, please advise me of the specific legal basis for any denial or redaction as required under OPRA. If responsive records are maintained electronically, they may be provided in electronic form.
