Under the authority granted by the New Jersey Open Public Records Act (N.J.S.A. 47:1A-1 et seq.), I am formally requesting the release of the below-mentioned body-worn camera and dashcam footage pertaining to the specific incident outlined below:

1. Officer Middleton, Louis[526] = bodycam + Dashcam (patrol unit)
2. Ptl Talavera = bodycam
3. Ptl Puchacz = bodycam

This request encompasses unedited footage from the above-mentioned officer's body-worn cameras and patrol vehicle dashcams involved in the referenced incident.

Effective September 3, 2024, Governor Murphy had signed into law significant OPRA reform legislation. Changes range from new exemptions to new processes for requestors and custodians. Attached you will find a copy of the new adopted Model OPRA request form which also can be located on our website under the Clerk’s Office and/or Police Records Bureau. We recommend to use the form to ensure full compliance however written equivalents are still allowed via email but must include all components required by NJSA 47:1A-5(f), (g).
