Records on former inmate J. Lyons DOB 7/12/01
Dear New Jersey Department of Corrections,
I represent former DOC inmate Joel Lyons, DOB 7/12/01, SBI #305600H. I have attempted to request, with valid authorization, my client's administrative and institutional records directly at the DOC Request Records email address, but was provided only an Incarceration History, with notice that the other information I seek requires an OPRA request. Thus, this OPRA request for Mr. Lyons' full incarceration file, including (but not limited to) records pertaining to:
Intake, classification, medical, mental health, testing, assessment, work assignment and supervision, recreation, legal action, transfer, transport, complaint, grievance, commendation, visitation, disciplinary action, transfer, and release.
Please acknowledge receipt of this message and advise an expected date of production.
