- Any municipal or health violations on the property, outside fines, judgments and liens on the property.

1. I HAVE or HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.
2. I, or another person, WILL or WILL NOT use the requested government records for a commercial purpose.
3. I AM or AM NOT seeking records in connection with a legal proceeding
If you answer "yes" to #3, you are required by OPRA to provide the docket number for the legal proceeding.
You request will not be processed until answers to the all the above questions are provided.
