You have not used the approved OPRA form, which is allowed, but you have also not provided all the ID information the form requires by stated law (Name, Adress, Email , phone number.
If you do not provide this information the records custodian can consider your request as anonymous and or reject it. Please provide the requested info or advise this is an anonymous request.
