Dear West Milford Township,
Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-5(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.
I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

1. Copy of all permits and approvals.
2. List of all open permits.
3. Property card
4. Any current or pending assessment on the property, if any.
5. Copy of survey if on file.
6. Any notices on record, if any, of offside conditions that pertain to this property.

The OPRA request submitted is received and can be processed for Township of West Milford records.
Effective July 8, 2026 a Passaic County OPRA Form must be completed and submitted for any Health Department records.
The Township of West Milford no longer maintains these records.
