Kindly be advised, requestors are not required to use an OPRA request
form; however, a written equivalent not containing the form requirements
of N.J.S.A. 47:1A-5(f) and N.J.S.A. 47:1A-5(g) may be denied by a
custodian. Attached for your reference is the most up to date OPRA Form to
be filled out by you or you may contain the requirements of the statute
noted above in an updated email request. You may also use the option of
entering your request through the portal via link : [1]Open Public
