OPRA Request for MVA 12/16/2020
Dear Lyndhurst Township,
Please be advised that I represent N. Alzaza , for injuries resulting from a motor vehicle accident that occurred on 12/16/2020, case number 20-16669.
Please provide the following:
(1) A complete copy of the entire investigation file regarding this incident;
(2) All surveillance video and/or traffic camera video at Riverside Avenue & Stuyvesant Avenue.
(3) All police dash camera videos including the audio recordings related to this incident;
(4) All police body camera videos including the audio recordings related to this incident;
(5) All notes taken by the responding police officers including the name, address, and telephone numbers of all witnesses to this incident;
(6) All photographs and/or videos regarding this incident;
(7) All reports from the Police Department, Fire Department regarding this incident;
(8) All CAD reports regarding this incident;
(9) All 911 phone call audio recordings describing and/or mentioning this incident to the 911 dispatcher, police, and/or fire department.

With regard to your OPRA request dated February 24, 2021, please see
attached responsive documents. Note that these are the only responsive
documents the Lyndhurst Police Department has on hand with regard to the
incident in question. The Lyndhurst Fire Department did not respond to
this motor vehicle accident.
The Body Worn Camera footage requested is available for pick up at our
office. There is a $1.00 charge for this disc, due upon receipt. If you
would like this disc mailed to you please indicate so and I will advise
the cost of shipping plus the $1.00 disc fee, which would need to be
received in our office prior to mailing.

I received an email to "explain part of your OPRA request. You will only get an answer to your request if you follow up with the clarification."
I had replied to your last message with regard to Body Cam footage was ready for pick up at your office, but I asked if the disc could be mailed to Brach Eichler LLC with address attached. Kindly state if that is possible and to state the full amount of disc and postage. Then we can issue the invoice to accounting so they may cut a check.
