This is a request for public records made under OPRA and the common law right of access. I am not required to fill out an official form. Please acknowledge receipt of this message.

*Certificates of Continued Occupancy
*Certificates of Approval
*Certificates of Compliance
*Please provide records for all submissions made within the last 60 days, limited to commercial properties or spaces, in a spreadsheet or CSV format.

Dear M. Salem,
Pursuant to the law governing OPRA, “[i]f a letter or an email from a
requestor does not include all of the information required on the adopted
form, the custodian may deny the record request.” N.J.S.A. 47:1A-5(f).
Please note that OPRA further provides – “A party to a legal proceeding
may not request a government record if the record sought is the subject of
a court order, including a pending discovery request, and a custodian
shall not be required to complete such a request. The requestor shall be
required to certify whether the government record is being sought in
connection with a legal proceeding and identify the proceeding for the
request to be fulfilled.” N.J.S.A. 47:1A-5(g).
Kindly provide your responses to the following:
Under penalty of N.J.S.A. 2C:28-3, please certify the following:
1. Have you been convicted of any indictable offense under the laws of
New Jersey, any other state, or the United States?
2. Are you, or another person, using the requested government records
for a commercial purpose?
3. Are you seeking records in connection with a legal proceeding?
Please note that the response deadline will not commence until we are in
receipt of your answers.
