You are correct that you are not required to fill out an official OPRA
form.  However, on June 5, 2024, Governor Murphy signed into law
legislation to modernize OPRA.  The new law continues past practice of not
requiring OPRA requests to be submitted on a form, however, the law does
require that if the official form is not used, then the request must
include a statement answering the following questions:
Under penalty of N.J.S.A. 2C:28-3, I certify that
1.  I HAVE / HAVE NOT been convicted of any indictable offense under the
laws of New Jersey, any other state, or the United States;
2.  I, or another person, WILL / WILL NOT use the requested government
records for a commercial purpose;
3.  I AM / AM NOT seeking records in connection with a legal proceeding.
Please resubmit your request with the appropriate answers to the above
questions.
