NEW Open Public Request (OPRA) Form – the new form must be used for any requests received on or after 9/9/2024. Please note the addition of questions 2 & 3 in the top section which has been added by the Government Records Council. ALL sections must be completed, or your request will be deemed incomplete and be denied. The new form is attached.
