Effective 7/1/2021 commissions were removed from the Medical, Rx, Dental
and Vision plans and Roselle BOE signed a Fee Agreement for $190,000 per
year. See attached and below:
• Fee Agreement - Attached/2 years beginning 7/1/2021
• Pdf (3 pages) RBOE – Health Annual Plan Cost (spreadsheets reflect
annual costs based on estimated headcounts; I’m also including signed
rate sheets; your discretion to send)
◦ Horizon Medical/Rx: $10,680,819
◦ Delta Dental: $320,820
◦ VSP/Vision (100% voluntary): $21,222
Rate sheets from carriers; reminder the 3 PPO plans with Delta were in a
2 year rate guarantee eff 2021
• Horizon – 2 versions as there was a correction to the EPO rate and Rx
10/20. Since the rates were sent for signature via docusign, I could
not merge pages.
• Delta – signed PPO rate sheet from 2021 as PPO rates had a 2 year rate
guarantee – only applies to PPO rates
◦ Flagship rates eff 7/1/2022
• VSP rates
