Entire file, insurance information
Dear Chartertech High School For The Performing Arts,
Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-6(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.
I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.
I WILL NOT use the requested government records for a commercial purpose.
I AM NOT seeking records in connection with a legal proceeding.
Records requested: The requestor's entire personnel file. Your contract of insurance with your liability carrier.
My preferred delivery method for response(s) to this request is by E-mail as attachments. Please confirm you have received this request. If you are not the custodian of records, please forward my request to that person and provide their email address to me for future reference.

Dear Mr. Dalnoky:
We are in receipt of your request for: "The requestor's entire personnel
file." "Your contract of insurance with your liability carrier."
In regards to "the requestor's entire personnel file" we are granting the
request based on the exception that release of information is "authorized
by an individual in interest." We are requesting an extension of time to
obtain the records, as the OPRA request was sent after the school was
dismissed for winter break, starting at 3:00 PM on Friday, December 19,
2025 through Friday, January 2, 2026. All personnel records are
maintained in paper form and secured in limited access files on site. The
records cannot be obtained until the school reopens on Monday, January 5,
2026 and authorized personnel will be able to access them. All records
requested will be forwarded on that date.
In regards to "your contract of insurance with your liability carrier" we
have granted that request. I am attaching the requested document here, as
it is electronically accessible.
