Carbon Health is paid to Central Jersey Urgent Care. Attached is the payments we made in the last 10 years. The first payment was made in September of 2018. Virtua Medical payment start in February 2016 until November 2018. These are payments for employee physicals and workers compensation, not Emergency Medical Services.

The attached information completes your request at this time.
