Dear Long Branch City,
Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-6(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.
I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.
I WILL NOT use the requested government records for a commercial purpose.
I AM NOT seeking records in connection with a legal proceeding.

Code Violation List Request for 01/01/2024 to 12/31/2024. I kindly request a list of all UNRESOLVED or OPEN or CLOSED code violations (inclusive all ZONING and BUILDING code violations) for all properties in the city during this time period.
Please include:
- violation number
- descriptions of the violation
- property addresses
- block and lot
- violation dates
- official complainant and office
- penalty associated with violation
I also prefer for the document to be sent here in OPRA not in my email (sometimes it does not appear in my inbox).
Please confirm you have received this request. If you are not the custodian of records, please forward my request to that person and provide their email address to me for future reference.

9. Do requestors have to use the official Open Public Records Act (OPRA) form?
Yes and no. In the case of Renna v. County of Union, 407 NJ.Super. 230,, (App. Div. 2009), the Court said: "We hold that all requests for OPRA records must be in writing; that such requests shall utilize the forms provided by the custodian of the records; however, no custodian shall withhold such records if the written request for such records, not presented on the official form, contains the requisite information prescribed in NJ.S.A. 47:1A-5(f)."
The Court stated that requests should be made on the form provided by the custodian of records, but that, so long as all the required information is present, failure to use the form cannot result in a refusal to withhold the records.
The Court went on to say: "In the event the submitted writing, exclusive of the form, is insufficient for the custodian to fulfill its statutory responsibility, such as the record may not be subject to OPRA or the request does not contain sufficient information, the custodian may require the requester to complete the official form."
