While the Open Public Records Act does not require requestors to complete an official form, N.J.S. 47:1A-1 et seq. requires requestors to certify whether:

• The government record will be used by that requestor or another person for a commercial purpose
• The government record is being sought in connection with a legal proceeding and identify the proceeding for the request to be fulfilled
• You've been convicted of any indictable offense under the laws of this State, any other state or the United States
