former municipal employee

compensations such as stipends, allowances, bonuses, reimbursements,
payments in lieu of health benefits, transportation, etc.

My preferred delivery method for response(s) to this request is by
E-mail as an Excel spreadsheet attachment.  If you have any questions

provide their email address to me for future reference.
