This office received a request for information from you on August 17, 2026 to
which we assigned the above referenced number used to log requests for
information under the Open Public Records Act (OPRA). 

You were notified that effective July 8, 2026 a Passaic County OPRA Form must be
completed and submitted for any Health Department records.
