For the period from January 1, 2018 to the present date,

(1) true copies of all applications and/or escrow payments submitted to the Township of Mansfield by Cellco Partnership, AT&T Mobility, Verizon Wireless, T-Mobile US, Sprint Corporation, Charter Communications, SpectrumCo LLC, Cox TMI Wireless, LLC (also known as Cox Wireless), Bright House Networks, LLC, Cox Communications, Cleareon Fiber Networks, LLC, Competitive Local Exchange Carrier (CLEC), or any agent or affiliate thereof, for approvals for new and/or collocation of antennas and/or any other telecommunications equipment in the public right-of-way. Please note that antennas and/or telecommunications equipment referred to herein are often referred to by several different names. Such names include: "Small Cell" antennas, "small cell nodes," "small node antennas," "Distributed Antenna Systems (DAS)," "5G or 5th Generation Technology," "Wireless Network Facilities" (WTF), "5G Deployment," etc.;

(2) true copies of all public records of communications between the Township of Mansfield and Cellco Partnership, AT&T Mobility, Verizon Wireless, T-Mobile US, Sprint Corporation, Charter Communications, SpectrumCo LLC, Cox TMI Wireless, LLC (also known as Cox Wireless), Bright House Networks, LLC, Cox Communications, Cleareon Fiber Networks, LLC, Competitive Local Exchange Carrier (CLEC), or any agent or affiliate thereof; and

(3) true copies of all public records of FCC compliance reports submitted to the Township of Mansfield by Cellco Partnership, AT&T Mobility, Verizon Wireless, T-Mobile US, Sprint Corporation, Charter Communications, SpectrumCo LLC, Cox TMI Wireless, LLC (also known as Cox Wireless), Bright House Networks, LLC, Cox Communications, Cleareon Fiber Networks, LLC, Competitive Local Exchange Carrier (CLEC), or any agent or affiliate thereof.
