Good morning and thank you for your request. Last year, 2024, the Governor
signed a new OPRA law. As one of the changes, a new OPRA form was created.
This form is mandated state-wide.

I have attached the new OPRA form for your reference.

There are three questions which need to be answered before this request
can be processed. The seven-day response time would not begin until those
three questions are answered.

Please answer the following 3 questions via email, and your request will
be processed:

 1. I HAVE / HAVE NOT been convicted of any indictable offense under
the laws of New Jersey, any other state, or the United States.
 2. I, or another person, WILL / WILL NOT use the requested government
records for a commercial purpose.
 3. I AM / AM NOT seeking records in connection with a legal proceeding.

Note: if you confirmed above that the records sought are in connection
with a legal proceeding, identification of that proceeding is required
below:
