Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-6(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

Specifically, I am requesting the following records for the period of 12/31/2026 - 1/2/2026

Traffic signal maintenance logs, including routine inspections, corrective maintenance, and emergency responses.

Controller event logs, including but not limited to:

Conflict Monitor (MMU/CMU) logs

Fault, alarm, or error reports

Power interruptions, restarts, or cabinet door openings

Signal timing plans and phasing diagrams in effect on the above date.

Work orders, service tickets, or dispatch records related to any reports, complaints, or maintenance activity at or near this intersection on the above date.

Any records of reported signal malfunctions or public complaints received by NJDOT regarding this intersection on the above date.
