This request is limited to employees only and does not seek:

Dependent information
Social Security numbers
Dates of birth
Home addresses
Medical or plan selection details

If the requested information exists in multiple records (such as insurance enrollment rosters, benefit summaries, or billing records), please provide the portions of those records that are responsive to this request.

If any portion of the records is exempt from disclosure, please redact only the exempt material and provide the remainder, as required by OPRA. If access is denied in whole or in part, please cite the specific statutory basis for each denial.
