Dear Dena Hrebenak,
You provided a response to a July 28, 2017 OPRA request stating two claims were filed in 2013, two claims were filed in 2014, and two claims were filed in 2015. Of those, you provided information that all claims were approved and paid with the exception of the two 2015 claims. This request is asking for the same info for 2016 and 2017. Further, it is asking for the claim outcomes for those claims from 2013-2017 claims and the amount paid out for those same claims.

Dear Dena Hrebenak,
I am still awaiting response prior to you destroying any records at the end of October. I previously provided clarification for which you have not responded. Again, this is an OPRA request for the detailed past claim information against the township that you already provided total summary claim data for (2 in 2013, 2 in 2014, 2 in 2015). Please respond ASAP.
