Good afternoon Lingyan Quan

          I have done some research to provide the requested documents.  I
was not able to locate a signed registration application, but that may be
due to us converting to an electronic application process in August of
2015.  However, I am able to provide the payment associated with the 2016
registration application process, as well as the resulting registration
document – both are attached to this email.

 

          This request is deemed satisfied/closed.
