Dear Middlesex County Utilities Authority,

This is a request for public records made under OPRA and the common law
right of access. I am not required to fill out an official form. Please
acknowledge receipt of this message.
Records requested:

Please provide all records related to installation, location,
decommissioning, or removal of all sewer/septic for 223 Austin Ave in Old
Bridge, NJ 08857 This should include any site plans, surveys, permit
applications, and inspection reports.

Please also provide dates/documentation for when the property was
connected to public sewer and any applications, permits, inspection
reports, and site plans related to such.

Lastly, please provide any and all information related to any gray water
tanks on said property.
