Service request forms
Dear Monmouth County,
Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-6(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.
I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.
I WILL NOT use the requested government records for a commercial purpose.
I AM NOT seeking records in connection with a legal proceeding.
Records requested:
I would like a copy of all service request forms from January 1, 2024 through March 16, 2025
My preferred delivery method for response(s) to this request is by E-mail as attachments. Please confirm you have received this request. If you are not the custodian of records, please forward my request to that person and provide their email address to me for future reference.
Yours faithfully,
William Bailey
I will be out of the office with limited access to email. If this is

Please notify the sender by return e-mail if you have received this
message in error.
Please be advised that your request as written is overly broad, unclear and not a request for specific identifiable government records.
I direct your attention to the GRC opinion re: Complaint No. 2008-51 which provides in relevant part:
“[w]hile OPRA provides an alternative means of access to government documents not otherwise exempted from its reach, it is not intended as a research tool litigants may use to force government officials to identify and siphon useful information. Rather, OPRA simply operates to make identifiable government records ‘readily accessible for inspection, copying or examination.” MAG Entertainment, LLC v. Division of Alcohol Beverage Control, 375 N.J. Super. 534, 546 (App. Div. 2005). The Court further held that “[u]nder OPRA, agencies are required to disclose only ‘identifiable’ government records not otherwise exempt. Wholesale requests for general information to be analyzed, collated and compiled by the responding government entity are not encompassed therein. In short, OPRA does not countenance open-ended searches of an agency’s files. Emphasis supplied. Page 5.
Furthermore, “a requestor must specifically describe the document sought because OPRA operates to make identifiable government records ‘accessible.’ As such, a proper request under OPRA must identify with reasonable clarity those documents that are desired, and a party cannot satisfy this requirement by simply requesting all of an agency’s documents.” Page 6. Requests for “any and all documents” which do not provide identifiers other than broad generic descriptions do not satisfy this requirement. Custodians are also not required to conduct research or create new records in response to an OPRA request and are permitted to deny access for such requests.
Please provide clarification in writing and identify the specific government records you seek, and the County will make every effort to provide any documents responsive to your request.

Dear Brown, Tamara,
My request is NOT overly broad, it is very specific. The government documents that I am looking for are the service request forms and for specific dates. GRC rulings are not court opinions but the caselaw, wronko VS Jackson twp is. Fulfill my request as written
Yours sincerely,
William Bailey
Dear Sir,
Please be advised, "The County of Monmouth does not recognize the term "service request form".

Dear Monmouth County Public Records,
This is NOT a new request. I am simply clarifying the exact record I want, since you are too inept to know what I wanted.
Yours sincerely,
William Bailey

Dear Monmouth County Public Records,
You sent me a blank form, the same one I sent you to show you exactly what I wanted. However, you didn't fulfill my request of the ones that are filled out. From januaryb1, 2024. Through March 16, 2025. If you want to play games, I have more games than milton Bradley. Either you furnish my request or the games will begin and you won't like it.
Yours sincerely,
William Bailey
