Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-5(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

I WILL NOT use the requested government records for a commercial purpose.

I AM NOT seeking records in connection with a legal proceeding.

Records requested, for the property at 11 Martintown Road, Woodbine Borough, NJ 08270 (Block 41, Lot 4; APN 1600041000000004), from the Cape May County Health Department / Environmental Services (septic program under CEHA) or any other county custodian holding them:

1. All individual subsurface sewage disposal system (septic) permits, applications, and approvals, any year.
2. All engineering designs, soil logs, percolation tests, and as-built drawings.
3. All installation, alteration, repair, or expansion permits and final inspection/approval records.
4. All abandonment or decommissioning records (septic or cesspool).
5. Any cesspool records and any malfunction, failure, complaint, or enforcement records.
6. Any record indicating the property is NOT served by an on-site system (for example, sewer-connection documentation), if such a record exists.

Preferred delivery: electronic copies via this request. If any fee beyond nominal copy charges (over approximately $10) would apply, please advise before processing.
