Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-5(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

I WILL NOT use the requested government records for a commercial purpose.

I AM NOT seeking records in connection with a legal proceeding.

Records requested:

Request copies of the following records relating to the Food Service Management Company Request for Proposals (RFP) for the applicable school year:
1. The complete Request for Proposals (RFP) package as issued to all prospective proposers, including all specifications, instructions, appendices, forms, exhibits, addenda, amendments, and attachments.
2. All complete proposals received in response to the RFP, including all quotations, fees, pricing, etc.
3. All evaluation committee records, including but not limited to:
o evaluation score sheets;
o scoring matrices;
o bid calculation worksheets;
o pricing comparison spreadsheets;
o consensus scoring sheets;
o evaluator notes, to the extent they constitute government records;
o ranking sheets;
o tabulation sheets;
o evaluation summaries; and
o any spreadsheets or documents used to calculate final scores or rankings.
4. Any document identifying the evaluation criteria, point allocations, weighting methodology, or formulas used to evaluate proposals.
5. The final recommendation prepared by or for the evaluation committee.
6. Any memorandum, report, or written recommendation submitted to the Board of Education recommending the successful proposer.
7. Any records reflecting the final scoring or ranking of each proposer.
If any responsive records are withheld or redacted, please identify each withheld record and state the specific statutory exemption relied upon for the denial.
