As discussed at our last meeting, here is a draft of the RFP for Emergency Medical Services for Allentown & Upper Freehold, the items I have highlighted in red require clarification. They are as follows:

• Page 1 – Additional Info – Use Allentown or UFT and Allentown?
• Page 8 – Security – Mandatory Submissions  Not sure what submissions are required, checked them all.
• Page 10 – Use both websites or one?
• Page 13 – Who’s tax exempt # do we use? I need the number.
• Page 25 – Stand by, do we want to include it?
• Page 25 – Subscription Agreement, do we want to include it as I have it? Or delete entirely?

Please take a look at the rest of the document and please make any changes/revisions you see fit. Please note – I will do a spell check, formatting, etc. prior to sending out final document to everyone.
