Dear Maple Shade Township,

This is a request for public records made under OPRA and the common law right of access. I am not required to fill out an official form. Please acknowledge receipt of this message.

Records requested: Police Report for
2012-15167. Incident date is 09/20/2012. I am the subject of the report and I need to show that the incident resulted in a citation, not an actual arrest. This is a time sensitive request as I need to provide this as part of a foster care application to be able to be foster parents.
