Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-5(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

I WILL NOT use the requested government records for a commercial purpose.

I AM NOT seeking records in connection with a legal proceeding.

I am writing to request an internal review of Voorhees Township's handling of my OPRA request 'Victim request for materials involving Virtua Voorhees Hospital staff harassing and assaulting myself the patient 12-12-25 to 12-13-25'.

I have requested the information connected to this Assault and it has not been answered within the required time out of compliance with the OPRA laws. I was assaulted, harassed, torched and seriously injured in this matter to include permanent spinal and permanent brain damage. Up to this point the department has with held the information from the investigation purposefully which seems to be an attempt the hospital in this illegal activity which is a violation of the law. None of the information in the police report included the perpetrators which admitted assaulting me the patient to the officers at scene but were not included in the report for 12-12-25. Again I request the all materials involving the assaults on 12-12-25 and 12-13-25 involving Virtua Hospital and myself Nickoles Stankowski. This is to include all body cams footage from every officer at the scene, this is to include all paperwork, notes, follow up meetings, emails, audio recording and 911 calls involving this case. Not doing so will lead to an official complaint to the count and state and FBI against the TWP of Voorhees for not releasing said information as well as press release to local and national media. It’s again very concerning there has not even been any communication on this matter. Also it’s very concerning that your records department first gave a redacted copy of the 12-13-25 report hiding the perpetrators information which I have copy of. Then ended up getting an un redacted copy after informing them of how inappropriate it was to hide information from the victim.
Also highly disturbing as a retired state law enforcement officer to see this lack of action. Please follow up.

I have also been assured that the investigations are closed and their are no current legal proceedings connected to these records.

I am writing to request an internal review of Voorhees Township's handling of my OPRA request 'Request for Record from Voorhees Police Department.

I submitted this request multiple times now and have not got a reply back at all. I was told by a SGT in the department that the cases are no longer under investigation. I’m requesting this information as the police reports were incomplete and did not list all tge facts that were recorded including admission of guilt from one of the perpetrators admitting to the staff assualt in me on body cam while I was listening to the one patrolman talking to the security supervisor who admitted his staff assaulted me and then the patrolman not listing that in the report nor the details of who assaulted me and there was a witness to the start of the incident that I requested be interviewed and I was not told if done so. Also, I repeated several times on Body Came footage what happened to me for a lot of the details to be left out of the report. Please follow up and provide documentation accordance with the law and policies and provisions according to OPRA. Thank you for your help.

Please be advised that the responsive records have been reviewed and
certain information has been redacted in accordance with the applicable
provisions of the New Jersey Open Public Records Act (OPRA) and other
applicable laws.

The following information was redacted:

• The victim's name and other personal identifying information pursuant
to N.J.S.A. 47:1A-1.1 and applicable privacy protections, including
Daniel’s Law, if applicable.
• Information relating to the physician where disclosure could reveal
protected medical treatment information or where another applicable
exemption applies.
• Medical diagnoses, examination findings, and treatment details
pursuant to OPRA exemptions concerning confidential medical
information.

Additionally, please note that the body-worn camera footage was reviewed
and processed separately. Any required redactions to the footage were
applied in accordance with applicable statutes, policies, and privacy
protections.

The above redactions were made to protect confidential information while
providing the portions of the records that are subject to disclosure under
OPRA.

Pursuant to the New Jersey Open Public Records Act, if you disagree with
any redactions or believe additional records should have been disclosed,
you may challenge this determination by filing a complaint with the
Government Records Council (GRC) or by filing an action in the Superior
