Dear State Police representatives,

To better understand all perspectives of an incident that occurred, I am requesting info on the following:

Under Penalty of N.J.S.A 2C:28-3, I certify that:
1. I Have not been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.
2. I, or another person, WILL NOT use the requested government records for a commercial purpose.
3. I AM NOT seeking records in connection with a legal proceeding.

Specifically, I require the primary Detailed Narrative Report. To ensure a complete record, please also include all supplementary reports from every officer present at the scene.

Furthermore, provide any associated arrest summaries and investigative notes. If any portion of these records is withheld, please cite the specific statutory exemption authorizing the redaction. Please inform me of the expected timeline for delivery.
