Dear New Jersey Department of Health, Division of Epidemiology, Environmental, and Occupational Health,

This is a request for public records made under OPRA and the common law right of access. I am not required to fill out an official form. Please acknowledge receipt of this message.

Under Public Law 116-136, § 18115(a), all positive Covid-19 cases are sent to the (STLT) public health departments as described in the guidance.

In OSHA Complaint No. 1594673, the NJ Bergen Regional Health Commission had the business in their jurisdiction confirm business positive Covid-19 records connected only to that complaint.

I'm requesting confirmed employee-positive Covid-19 records & other related Covid-19 records from 11/1/21 to 10/1/22 from the business located at 100 Redneck Avenue in Moonachie, NJ.

Lastly, I requested the same from Bergen County; their response is below.

Yours faithfully,

Eli R. King

D'Aloia, Christina A., Bergen County May 23, 2023
