Records Request for DCA on DFS Certifications
Dear New Jersey Department of Community Affairs, Division of Fire Safety,
This is a request for public records made under OPRA and the common law right of access. I am not required to fill out an official form. Please acknowledge receipt of this message.
Records requested:
Please provide the applications with the supporting documentation (submitted by the applicant or on behalf of the applicant) for any certifications and/or licenses issued or denied for the following persons: Mario Batista, Andrew Zdrodowski, and Jason Patino.
This request shall include any documents submitted by the applicant or on behalf of the applicants and or any documentation or communications from the Divison of Fire Safety to the applicants listed above for the following certifications and/or licenses: Firefighter 1, Firefighter 2, Hazardous Materials Awareness, Operations, Technician & or Specialist, Incident Management Level 1, 2 & 3, Fire Officer, Fire Investigator, Fire Police Officer, Fire Inspector, Fire Official, Fire Instructor (Levels 1 and 2) and Drill Ground Instructor.
Since the New Jersey Department of Community Affairs, Division of Fire Safety has refused to answer said OPRA request and it has 2 months, please obtain the information and provide a response. thank you

The Division reviewed your request and determined that it had the current and historical certification information on file for Jason Patino, Andrew Zdrodowski and Mario Batista. Accordingly, the Division is forwarding these records in PDF format.
Please be advised that all personal identifying information has been redacted from these records (i.e., home address, social security number, date of birth, home and cell phone number and personal email address).
Be further advised that the Division does not possess the actual certification applications. In accordance with the State's and Division's records retention policies and procedures, these have been either destroyed or archived.
Upon transmittal of this email and attachments to you, the Division will deem your request as closed.
