Records Lobosco Insurance Group Contracts, Tort Claims Administration Transition, Authorized Recipient of Tort Claims, & Appointment of Custodian Colleen Carver
Dear North Haledon Borough,
Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-5(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.
I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.
I WILL NOT use the requested government records for a commercial purpose.
I AM NOT seeking records in connection with a legal proceeding.
Records requested:
Pursuant to the New Jersey Open Public Records Act (OPRA), I request copies of the following records:
1. All contracts, agreements, renewals, amendments, extensions, or termination documents relating to The Lobosco Insurance Group’s services to the Borough of North Haledon from January 1, 2024, to the present.
2. All contracts, agreements, appointment documents, renewals, or amendments for any entity that assumed responsibility for the Borough’s risk management, insurance brokerage, tort claims administration, or claims administration during that same period.
3. All resolutions, ordinances, meeting agendas, meeting minutes, or other official records approving, renewing, terminating, or changing those services, including the effective date of any transition.
4. Any records identifying the entity authorized to receive notices of claim under the New Jersey Tort Claims Act as of September 13, 2025.
5. Any broker-of-record letters, risk management consultant appointment records, Joint Insurance Fund appointment records, or similar documents in effect as of September 13, 2025.
6. All records appointing, designating, or authorizing Colleen Carver as the Borough’s Custodian of Government Records (OPRA Custodian), including any resolution, ordinance, appointment letter, job description, personnel action, or other official record establishing that designation and its effective date.
7. If Colleen Carver replaced a prior Custodian of Government Records, all records reflecting that transition, including the effective date and any resolution or appointment document.
If no responsive records exist for any item, please indicate that no responsive records were found.
