Dear Hawthorne Borough,

Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-5(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

I WILL NOT use the requested government records for a commercial purpose.

I AM seeking records in connection with a legal proceeding.

I AM seeking records under the common law.

Records requested:
Pursuant to the New Jersey Open Public Records Act, please provide electronic copies of any and all police reports created, received, or maintained by the Borough of Hawthorne and/or the Hawthorne Police Department concerning the October 9, 2025 police response / operation involving me.

This request includes, to the extent they exist:

incident reports;
investigation reports;
supplementary reports;
offense reports;
call-for-service reports;
CAD or dispatch event reports / printouts;
any report, narrative, or record reflecting drone deployment, drone assistance, aerial surveillance, or inter-agency assistance provided on October 9, 2025 in connection with that incident;
any report identifying the officers, units, supervisors, or equipment involved in Hawthorne’s participation in that incident.

If any responsive record is withheld or redacted, please identify the record and state the specific legal basis for the withholding or redaction.

If this request is deemed overly broad, please construe it narrowly to seek the specific incident report(s), supplemental report(s), and CAD / dispatch record(s) for Hawthorne’s participation in the October 9, 2025 incident involving me.

Please provide all responsive records in electronic form.
