Dear Brick Township,

This is a request for public records made under OPRA and the common law right of access. Please acknowledge receipt of this message. I respectfully request the following records;

1. All documents and written communications related to the subject climbing wall, including, but not limited to, all identifying information about the climbing wall: designer, manufacturer, seller, model and serial numbers;
2. All documents and written communications regarding the sale, purchase, installation, maintenance, repair, inspection, and complaints regarding the subject climbing wall from the date of purchase through April 18, 2023;
3. The contact information of all entities and persons who have had any involvement in the design, manufacture, sale, purchase, installation, maintenance, inspection, and repair of the subject climbing wall;
4. Any incident reports regarding the subject climbing wall;
5. All incident reports involving all other persons who were injured, at any time, on the subject climbing wall at Osbornville Elementary School, from the date of installment to the present;
6. All documents and written communications from Brick Township School Board regarding the subject climbing wall.
